James – Brain Injury

James was 30 years old when he sustained a brain injury in a car accident. He was also left with some physical injuries, reduced mobility, poor speech and subsequently became dependent on his sister, who provided care and support to him.

Following a comprehensive case management assessment, James was introduced to support workers who assisted him to access local activities and increase his community presence. Specialist neurological clinicians assessed James and provided him with a goal orientated rehabilitation programme, focused on his wishes to live independently in the future.

James has developed positive relationships with his support and clinical team, and is provided with a bespoke rehabilitation and support package, enabling him to reduce his dependence on his sister and become more proactive in his personalised programme. James has successfully completed an initial trial of independent living in a self contained apartment with support and continues to engage with services provided to him.

James is focusing on a return to the workplace via a supported placement and plans to live in his own home, without the support of his sister. He continues to make progress with his mobility and speech skills, in addition to cooking meals for his sister, which he previously enjoyed.

James continues to work with his case manager, support workers and clinicians to review and adapt his goals in line with his abilities and wishes.
